Replacement lenses

Have anyone seen this on a new pair of prizm lenses and does this info usually come in the box with replacement lenses?

I’ve never seen that checkered pattern on lenses. I’ve had lenses come with the paperwork

That checkered pattern on the lenses looks like it’s from the foam pouch. I’ve seen that on old straightlink lenses before. It comes off with some lens cleaner spray.
That info packet comes with all the lenses nowadays.

Yep, in a box and also in a ziploc baggie

Clean it with dishwashing liquid

It will probably be a return for these and get a new pair.
